Oregon: Bulletin addresses WC claims closure

The Workers' Compensation Division issued Bulletin No. 139 (Rev.) on Nov. 2, 2009, regarding revised forms for closing workers' compensation claims under ORS 656.268, OAR 436-030, and OAR 436-060.

The revisions include removing instructions for completion and example language from the Notice of Closure; removing the worker's Social Security number from all forms; simplifying the language for completion instructions; changing the name of the type of closure from "order" to "notice"; adding language informing the worker of the right to request vocational assistance; replacing completion instructions with a list of the most commonly used notice types; removing the field to request eligibility review for preferred worker benefits; and changing the method of reporting claim closure timeliness. The bulletin attaches the updated forms to be used. Insurers are required to use the version of the forms attached as of Jan. 1, 2010. The bulletin replaces Bulletin No. 139 dated June 14, 2006.
