The Workers' Compensation Division issued Bulletin No. 310 (Rev.) on Feb. 8, regarding Spanish-language forms 801S, Report of Job Injury or Illness, and 3283S, A Guide for Workers Recently Hurt on the Job, to meet the needs of injured workers whose primary language is Spanish.

Whenever Form 801S is used by a worker to report a claim, the insurer or self-insured employer should translate the information into English and enter it on a standard Form 801. The division's copy of the translated 801 should be stapled to 801S and submitted along with the other required documents in OAR 436-060-0010. Requirements for Form 3283S are in OAR 436-060-0015. For more information, visit www.wcd.oregon.gov.
