The form is now available for use by sole corporate officers to exclude themselves from employee status for workers' compensation insurance purposes. Additionally, any independent contractor (regardless of trade) may be granted a clearance certificate. The Department of Revenue must approve the application. For a copy of the revised form, visit www.in.gov/wcb/2328.htm.
