The department added a seventh health care practice guideline to the regulations regarding lower extremities. Wording in the regulations was changed to include current dental terminology codes for dental offices. Language was added that requires an answer for surgery preauthorization requests. The regulations also required changes to the following forms: 1) health care provider application for certification; 2) request for utilization review; 3) physician's report of workers' compensation injury; and 4) employer's modified duty report. The utilization review request form no longer includes attorney information, adds room for a second health care provider, and streamlines the medical documentation requirements.
