Vermont: Numerous workers' comp forms might change
The Department of Labor proposed changes to forms.
These include the employer first report of injury, denial of benefits, reports of a fatal accident, notice and application for a hearing, notice of intent to change health care provider, certificate of dependency and concurrent employment, settlement agreement, agreement for temporary compensation, and denial or discontinuance of vocational rehabilitation.
