Ohio: Outpatient medication formulary may restrict some drugs
In cases of medical necessity supported by clinical documentation and evidence of need, the bureau may, with prior authorization, reimburse for new dosage forms or strengths for existing drugs on the formulary for a period not to exceed 180 days from the adjudication date of the first prescription for the requested drug. The formulary will go into effect on Feb. 1, 2012. The bureau also proposed changes to a rule regarding the pharmacy and therapeutics committee. The change provides that the committee will consist of not more than 13 or less than five voting members who must be licensed physicians and licensed pharmacists representing providers who provide care to injured workers. Subcommittee recommendations must be submitted to the committee. The bureau held a public hearing.
