When a physician named in a change of physician order passes away while actively treating the worker for a compensable injury or prior to releasing the worker from treatment without having made adequate provisions for the worker's continued treatment, the change of physician order becomes void. Under these circumstances or when the physician is known to the commission as no longer in practice, the change of physician order is no longer considered the worker's onetime change of physician.
